The aircraft crashed around 10:15 hours local about 35 miles east of Marathon, Florida. It was determined by the AIB that the crash was caused by pilot error. A speed that was too low with a shifting center of gravity forced the jet out of control. The pilot, Maj. Timothy B. Piccin, ejected safely.

The aircraft crashed at 20:50 hours local near the end of the runway, while on approach to the Tonopah Test Range Airfield, approximately 20 miles southeast of Tonopah. The aircraft was flying a night training mission over the restricted Nevada Test and Training Range when his AB was stuck. The pilot tried to make an emergency landing but the lights at the airfield didn't come on, so he tried it with his NVG's. Right before he came in, the runway lights came on and he was blinded. He then saw he was short and after he took off the NVG's he ejected safely. Pilot was Maj. David Epperson.

The aircraft crashed near Altinapa Dam of Muhsine village. About 5-10 minutes after it took off from the 3rd Main Jet Base in the province, the jet went into secondary engine control (SEC) mode. While gaining altitude and heading back to the base, a flameout occurred. The pilot tried to restart the engine twice but to no avail. Captain Ilker Ozdil, age 34, ejected 15 nautical miles from the base. He was slightly injured and taken to Meram Medical Faculty Hospital of Selcuk University by helicopter for treatment.

Tipped over on its side again this time while taxiing before take off for a Red Flag North sortie. During taxi roll the aircraft wandered behind a B-1B which was spooling up to move out onto the runway. The exhaust of the four engines on the B-1 was powerful enough to blow the aircraft up on two wheels on its right side. This aircraft had previously been on its same side when it went out of control just a few years earlier on February 6, 2003. Just like before the aircraft will be repaired. It is unclear yet if the B-1 applied too much thrust for ground taxiing or the F-16 was out of position.

Lost during takeoff from Balad AB, Iraq at 16.55 hours with the pilot managing to eject safely. Accident investigation found that the nose gear tire was at low pressure and maintenance failed to properly check the tire pressure. During takeoff roll the nose gear tire blew when the aircraft reached 150 mph. Pilot heard the pop and thought it was an engine failure so aborted the take-off even though at high speed and running out of runway. Shortly after the nose gear collapsed and due to sever lack of control the pilot (also a commander) elected to eject. Aircraft continued another 2000 feet until it went off the runway where it tumbled end-over-end coming to rest upside down. Fire erupted and the 20mm rounds cooked off.
