Fell to earth near Euskirchen, Germany after both crew members, Captain Dirk Claeys and Slt. Glenn Schevenels, ejected safely

Crashed on the Vliehors range in the Netherlands. The pilot, 1st Lieutenant Willem van Suydam, did not eject and was killed in the crash. It is believed to have been target fixation. This happened near Vliehors range, Frisian Islands
13 Jan 1992 [w/o] 84267 84-1267 USAF 184 TFG F-16C Block 25D Details
Was returning from a practice firing sortie of a M61A1 20mm cannon, and in the process of beginning touch-and-goes. Major David Ternes of the 127th FS was bringing the aircraft into McConnell AFB when he experienced trouble at 11:06 hours. Ternes was forced to eject at 150 feet and two miles from base. The aircraft tore through a house nearly hitting the sole occupant. Both people survived uninjured. Cause was a lack of engine response.

Crashed at the Utah Test and Training Range during a practice bombing run. This Desert Storm veteran was based at Hill AFB. The pilot was injured in the ejection. The aircraft was inadvertently entered IMC and then went into a steep dive from which the pilot was unable to recover.

Collided with a KC-135R (61-0284) of the 92nd Wing. The F-16 went down in the Pacific Ocean, 630 miles east of Atsugi JMSDF AB.
11 Feb 1992 [w/o] J-054 86-0054 RNLAF 315 sqn F-16A Block 15Y OCU Details
Crashed into an occupied area of Hengelo, Netherlands. Fortunately there were no injures and the pilot ejected safely. Shortly after takeoff, there were engine problems which resulted in the crash.

After experiencing a main generator failure, the aircraft landed safely at Leeuwarden AFB, Netherlands
03 Mar 1992 [w/o] 81704 81-0704 USAF 171 FS F-16A Block 15D ADF Details
Collided in mid-air with F-16A (81-0706) near Bad Axe, Michigan killing one of the pilots, Lieutenant Colonel Roy Allen Keyt.
