Fell to earth near Euskirchen, Germany after both crew members, Captain Dirk Claeys and Slt. Glenn Schevenels, ejected safely
10 Jan 1992 [ w/o] J-007 88- 0007 RNlAF 313 sqn F-16A Block 15 AG OCU Details
Crashed on the Vliehors range in the Netherlands. The pilot, 1st Lieutenant Willem van Suydam, did not eject and was killed in the crash. It is believed to have been target fixation. This happened near Vliehors range, Frisian Islands
13 Jan 1992 [ w/o] 84267 84- 1267 USAF 127 TFTS F-16C Block 25 D Details
Was returning from a practice firing sortie of a M61A1 20mm cannon, and in the process of beginning touch-and-goes. Major David Ternes of the 127th FS was bringing the aircraft into McConnell AFB when he experienced trouble at 11:06 hours. Ternes was forced to eject at 150 feet and two miles from base. The aircraft tore through a house nearly hitting the sole occupant. Both people survived uninjured. Cause was a lack of engine response.

Crashed at the Utah Test and Training Range during a practice bombing run. This Desert Storm veteran was based at Hill AFB. The pilot, 1st Lt. Brett Kulkarni, was injured in the ejection. The aircraft was inadvertently entered IMC and then went into a steep dive from which the pilot was unable to recover.
22 Jan 1992 [ w/o] 85496 85- 1496 USAF 13 FS F-16C Block 30 B Details
Collided with a KC-135R (61-0284) of the 92nd Wing after this aircraft performed a manouvre to avoid collision with another KC-135. The F-16 went down in the Pacific Ocean, 630 miles east of Atsugi JMSDF AB at 5:27 hours. Pilot, Captain John L. Dolan, ejected safely and was rescued by amphibious US-1A of 71st Koukuutai, JMSDF based at Atsugi AB.
11 Feb 1992 [ w/o] J-054 86- 0054 RNlAF 315 sqn F-16A Block 15 Y OCU Details
Crashed into an occupied area of Hengelo, Netherlands. Fortunately there were no injures and the pilot, 1st Lt. T.Saft, ejected safely. Shortly after takeoff, there were engine problems which resulted in the crash.

After experiencing a main generator failure, the aircraft landed safely at Leeuwarden AFB, Netherlands

Collided in mid-air with F-16A (81-0706) near Bad Axe, Michigan killing one of the pilots, Lieutenant Colonel Roy Allen Keyt. Collision was due to lack of situational awareness during combat air maneuvering.

Collided in mid-air with F-16A (81-0704) near Bad Axe, Michigan killing the pilot of 704, Lieutenant Colonel Roy Allen Keyt. Collision was due to lack of situational awareness during combat air maneuvering.
23 Mar 1992 [ w/o] 278 78- 0278 RNoAF 332 skv F-16A Block 5 Details
The aircraft was an early block 5, and was lost due to an engine failure at 18,000 feet. After three attempts to relight the engine, the pilot directed the aircraft away from populated areas and ejected at 3,000 feet. This happened near Banak AB, Norway
14 Apr 1992 [ act] 85490 85- 1490 USAF 13 FS F-16C Block 30 B AIB Report Details
Right after take-off from Misawa AB, Japan the aircraft suffered an inflight emergency. It jettisonned both external fuel tanks and landed uneventfully. Pilot was Lt. Clay W. Hall.
24 Apr 1992 [ w/o] 89110 89- 2110 USAF 308 FS F-16C Block 40 G Details
Three days prior to the accident the engine was installed by SSgt Al Lynch, SSgt Stewart Baily, SSGT Jim Biggs, Sgt Paula Forness, AIC Jerrod Whittaker and AIC Jason Wollever. After several delays, the aircraft was launched by SSGT Scott Ramsour. The aircraft taxied and went to EOR arm crew. On take-off the engine lost power and the pilot climbed to MSA to bring the aircraft back to the base. The aircraft made an emergency landing attempt and struck the approach overrun area 100 yards short of the runway on the right side. The main landing failed and the aircraft collapsed along with the nose landing gear. The aircraft came to rest between the taxiway and runway after sliding 1,500 feet. The pilot raised the canopy electrically and tried to climb out but couldn't since his G-suite hose was still connected. He was burned and recieved injuries to his back and spine but did survive after being rescued by the fire brigade. The pilot, Captain Mark A. Merkel, was initialy rescued by passing personel. The cause of the accident was determined to be a faulty augmentor fan control unit (AFT).
05 May 1992 [ w/o] 80610 80- 0610 USAF 186 FS F-16A Block 15 B ADF Details
Lost at Great Falls, Montana. Aircraft had experienced engine trouble during the flight. While landing and roll out, the engine accelerated out of control. The pilot ejected safely and the aircraft was a write off.
21 May 1992 [ w/o] 83071 83- 1071 USAF 704 FS F-16A Block 15 Q Details
Pilot ejected safely from his crashing aircraft while on a training mission. The aircraft came down in the Stony Military Operating Area, 250 miles from Anchorage, Alaska. The aircraft was on deployment from its home base of Bergstrom AFB, Texas.
26 May 1992 [ w/o] J-618 80- 3618 RNlAF 311 sqn F-16A Block 15 Details
Crashed during a training sortie 120 miles south of Goose Bay, Newfoundland. The pilot, Capt. Jac Jansen, ejected safely from the aircraft which was experiencing engine problems
