Collided in mid air with Mirage 2000EG (238), from the 332 Mira. The accident happened near the Island of Skiros (Pelagos) during a DACT exercise. The pilot from the Mirage, D Bratzios, was rescued but the F-16 pilot, Captain A Bridak was killed

Some time in 1995 shortly after engine start up, the engine experienced an auto acceleration malfunction resulting in an onboard fire. Aircraft was eventually repaired but exact date is still unknown.

During an engine check before take-off, a malfunction caused engine to engage full afterburner. The aircraft ran off the ramp and burned after sinking into a ditch and striking a tree. Location was Nea Ankhialos. The aircraft was taken to Hill AFB, Utah where it was repaired. It was flown again on October 12, 2001 by 514 Flight Test Squadron’s Major Jeffrey Clayton. This Greek aircraft spent 4 years at Hill AFB being repaired and is the most severely damaged F-16 to be repaired. Some parts had to be retooled as the block is older and some parts no longer made.

Engine cut while returning to its base at Nea Angchialos in company with another F-16. The engine cut at 8,000 ft, but after unsuccessfully starting the engine, the pilot ejected safely at about 1,000 to 1,400 ft above the sea just north of Skiros Island

At 12:48h local, this aircraft suffered a mid-air with a Turkish F-16C at 27,000 feet some 15km off the Greek island of Karpathos and consequently crashed some 21km off the coast of that island. Both were involved in a mock dogfight when one of the pilots made an error resulting in the collision. The Turkish pilot, 1st Lieutenant Halil Ibrahim Ozdemir ejected safely with only minor injuries, but the Greek pilot, Flight Lieutenant Konstantinos Iliakis, didn't survive this accident.

The jet participated in a nightly exercise in the Sporades archipelago with three other F-16s when it was lost by radars around 19.41h local time. Unfortunately the pilot, Flt. Lt. Athanasios Batsaras, 35 years, failed to eject. The cause of this fatal crash is still unknown but it is most likely to be attributed to spatial disorientation, more commonly known as pilots' vertigo.

Editors note:
A mishap that we recently discovered but where we lack the serial number to complete the list.
- 06/02/2003: A 111PM viper suffered engine problems right after the start at Nea Anghialos en tried to make a steep turn towards the runway. The pilot removed the external tanks but he couldn't make it to the runway. The aircraft exploded upon impact, but the pilot ejected safely.
