Aircraft had a leading edge malfuntion during flight which ripped off the majority of edge. It landed safely and was repaired. Need to find the date of this mishap.
15 Jan 1990 [w/o] 83089 83-1089 USAF 308 TFS F-16A Block 15R Details
Crashed near Homestead AFB, Florida. The aircraft was lost due to a burn-through in the engine and airframe. The pilot ejected when ATC personal saw a fire plume coming from the side of the aircraft. It was later determined that during a flame holder assembly change, a maintenance rag was left in the afterburner section causing a hot spot. The airforce F-16 units then began counting rags taken out on a daily basis. The aircraft was viewed with the vertical stabilizer above the water as it crashed in shallow water at the end of the runway.

This was reported as the first crash of an F-16A ADF. During a functional check flight, the engine stalled and the F-16 crashed 4 miles Northeast of Wendover, Utah. The pilot ejected safely.

Had a mid-air collision with F-16A (#78-0045) near Wendover, Utah. The pilot ejected safely.

23 Mar 1990 [act] FA-119 88-0044 F-16A Block 15AE OCU Details
Less than a month after delivery, the aircraft flies through some high voltage power cables in the valley of the Dordogne, Southern France. The pilot, 2nd squadron commander Cdt. Alain Letellier, succeeds in landing at Aulnat (near Clermont-Ferrand). The aircraft was heavily damaged on the belly, but was repaired

The mishap occurred at Kunsan AB, Korea due to an augmentor fuel tube failure at the flange weld and a resulting fire in the engine bay.
10 May 1990 [w/o] J-258 78-0258 RNLAF 322 sqn F-16A Block 15 Details
Collided in mid-air with F-16A (84-1358, J-358) thirteen kilometres west of Grand Lake, Canada. The aircraft were flying out of Goosebay. The pilot of this aircraft, Capt. Rein "Snatch" Sneek, ejected safely
10 May 1990 [w/o] J-358 84-1358 RNLAF 323 sqn F-16A Block 15W Details
Collided in mid-air with F-16A (78-0258, J-258) thirteen kilometres west of Grand Lake, Canada. The aircraft were flying out of Goosebay. The pilot of this aircraft, Capt. Leo "Moist" Vogt, was killed in the incident
