The pilot, 1st Lieutenant Ralph Cyr, tried to land in bad foggy weather during night at Shaw AFB, South Carolina. The aircraft impacted some pine trees not far from Shaw AFB, avoiding a trailer park. The pilot mistakenly saw street lights for the runway lights and was able to light the afterburner to avoid an impact in the trailer park but was already to low to avoid a crash. The lieutenant was killed in the accident.

This aircraft had a ADG failure on December 4, 1986 had to IFE into Goodyear airport. Student pilot in a four ship formation had to fly around a Beachcraft Bonanza on the runway, touched down and skidded onto the overrun at the end of the runway. The local airport Fire Department put out the fire in the brakes. Aircraft engine was replaced and flew back to Luke on December 8, 1986. No damage to the airframe.

Lost during landing after the engine failed. Major Boaz Nehorai was killed. After the aircraft suffered an engine failure, the pilot tried to land at an abondoned airstrip in de desert. He was able to land the F-16, but couldn't stop before the end of the runway. The aircraft tumbled up-side down and cought fire. The pilot stayed with the aircraft.
