Lost very shortly after takeoff from Volkel AB. After performing touch and goes it suffered an engine failure. The pilot, Jacob "Holy" Andringa, ejected safely. Accident was caused by a problem with the control of the inlet guide vianes.

Wrecked in a landing mishap at Volkel AB after gear collapsed. The crew of two were fine and did not eject. It was repaired at Woensdrecht from February to June of 2001 and returned to Volkel AB with 311 squadron by January 2002.

Control was lost in a dive after an unrecoverable engine stall during air combat manoeuvres on a sortie from Decimomannu. The incident took place over the Mediterranean, 18 miles off the western coast of Sardinia, Italy. The pilot, Lt. Roef Dieters, ejected safely.

Deering had jettisoned his external fuel tanks, but was forced to eject at 11:15 hours 15 miles northeast of the island of Terschelling. Deering was plucked out of the North Sea by the Leeuwarden SAR at 11:32 hours

Crashed in a residential area in the small town of Arum, Netherlands due to an unrecoverable engine stall. The pilot ejected safely, but a civilian on the ground was injured. The pilot was Captain Bob Verkroost who also ejected from F-16A J-625 on June 4th, 1988. This probably makes him the only Dutch pilot to eject twice from an F-16.

This aircraft crashed over the Waddenzee near the island of Vlieland around 15:00 hours local.The pilot was able to eject and was subsequently rescued by an AB-412SP helicopter. The cause of the accident is currently unknown, but the investigations all point to a crack in the diffuser case causing an engine failure.
