ACC releases Cannon F-16 crash investigation report

October 20, 1999 (by Lieven Dewitte) - Engine failure was the cause of an F-16C crash near Cannon AFB, N.M., July 12, according to the Accident Investigation Board report released today by Air Combat Command.
The pilot, Capt. Jason L. Marshall from Cannon's 523rd Fighter Squadron, ejected from the aircraft and sustained minor injuries. The aircraft was destroyed after impacting the ground in a military operating area about 85 nautical miles southeast of the base.

According to the report, Marshall reported an engine failure and flameout five minutes before impact. After going through critical action procedures for an engine failure and two attempts to restart the engine, he determined a successful glide and recovery was not possible and steered the aircraft toward an uninhabited area. Marshall then successfully ejected from the aircraft, 20 seconds before impact.

The engine shutdown, according to investigating officials, was caused by a catastrophic failure in the high pressure turbine assembly. The assembly failed when two blades separated due to fatigue.
