During a night sortie and a few minutes after take-off from Sargodha AB, the wingman informed his lead - Sqn.Ldr. Zafar Ahsan - and ground control that he is disoriented. They tried to help by repeatedly telling him to concentrate on the instruments but he crashed a few miles from Sarghoda, killing the pilot.

Destroyed at Ørland AFB, Norway, after being 'borrowed' by a ground crew member. The aircraft was stolen by a member of the ground crew and the aircraft crashed on a farm in the neighbourhood of the village Brekstad, close to Ørland AFB. The 28 year-old military took, in absence of his colleagues and pilots, control over the aircraft, started the engine and took off, to the surprise of the Norwegian crews. Five miles from the base he crashed after being airborne for 2 minutes and 5 seconds.

Crashed at Carswell AFB, Texas while doing a mock airfield attack. Three B-52's on the ground were slightly damaged. Both crew were killed. The front seat pilot was Captain Rob 'Pork' Tucker. The pilot in the back seat of the plane, Captain J. Bourne, was a pilot from VMFA-112. He was on an incentive flight, his first time to fly in a F-16. After doing a low pass over the runway at 350 to 400 knots, they intended to do a 90 degree bank, but instead did a 180 degree aileron role. This in turn was a very high "G" maneuver in which they slammed into the ground . The investigation determined G-lock. The plane disintegrated on impact between the runway and taxiway.

Engine turbine failed during a take-off roll sending pieces of the turbine blades through the fuel cell. Pilot aborted take off after seeing the fire light and shut down the engine. After the pilot opened the canopy and jumped out found the entire tail of the F-16 on fire. The aircraft was sent to depot but was never repaired.

Was restored to flyable status but suffered a blow out of the tail section due to a fire during a Cope Thunder exercise. The pilot - Lt. Rob Ray - dead-stick landed it. It took 8 months to repair. Rumors were the aft section of a previously damaged block 40 was used to replace the tail of this aircraft. It was later unofficially labeled a block 35.
