Lost near Tulelake, California. The pilot, 1st Lieutenant Stephen Taylor, a student pilot from the Vermont ANG, died in the crash. The aircraft was engaged in BVR with an instructor at the time of the crash. Spatial disorientation was most likely the cause.

The aircraft was completely destroyed in a crash. It impacted the ground near Union, Missouri due to bearing failure. The pilot ejected safely.
19 Mar 1983 [ act] 81779 81- 0779 USAF 612 TFS F-16A Block 15 G Details
Belly landed at Torrejon AB, Spain when the pilot forgot to lower landing gear. Aircraft, which was on a delivery flight, landed on three external tanks. Damage was considered minimal but was damaged when the recovery crane dropped it from roughly 4 feet during recovery. Because of that the aircraft had to have a spar replaced so a CLSS team flew in from Hill AFB, Utah and fixed the aircraft within a week.

Crashed in a south Georgia community, named Pearson. The crash killed the pilot, Lieutenant Colonel A. Stuart, and a civilian.
08 Feb 1985 [ w/o] 81808 81- 0808 USAF 614 TFS F-16A Block 15 H Details
Flew into the ground near Aviano AB, Italy. Pilot ejected safely. The cause was determined that both AOA heaters failed and ice formed on the probes, driving the probes full down. This caused the LE flaps to full down and the aircraft flipped on its back. The aircraft impacted a snow covered slop, inverted, and slid to a stop. The jet was enroute from Torrejon AB, Spain when the accident occured. During ejection, the pilot received a gash to his jaw from his parachute harness.
08 Jun 1991 [ w/o] 81814 81- 0814 USAF 111 FIS F-16B Block 15 D Details
Cause of the crash was a fuel shut-off valve, which is a safety device in case of a fire. The aircraft was flying from Holloman AFB, New Mexico to Ellington Field, Texas. The pilot felt a vibration followed by a bang which stopped the engine at an altitude of only 3,000 feet. The pilot re-light the engine and the same thing happened again. At this point the pilot, Captain Kenneth J. Gurry, ejected, because the aircraft was descending through 800 feet of altitude. The ejection occurred near League City, Texas. The ejection seat was repaired and used for training, then placed in the Texas Military Forces Museum.

This aircraft was involved in a mishap where the aircraft had an engine fire. Details to be confirmed, but photos after the accident show the aircraft appears fine on the exterior. Due to the age of the airframe, it was declared a write-off.
25 Sep 2003 [ act] 81821 81- 0821 USAF 148 FS F-16B Block 15 H AIB Report Details
The aircraft was damaged but repairable after hitting three power lines and destructing 10 power poles during a straight-in simulated flame-out approach (SFO) at Libby Army Airfield, Arizona. Both the instructor pilot and the student were unhurt, but the aircraft suffered some damage to the engine inlet, port strake, ram air diverter, nose gear, leading edge flap, flaperon, port horizontal stabilator and port speed brake. Cause of the incident was reported as lack of sufficient situational awareness by the instructor pilot.

Collided with F-16A (80-0597) over Indian Springs, Nevada killing Major Michael J. Lotti.

Mid-air collision with F-16B (82-1042) during a training sortie including BFM. The aircraft landed safely.

Lost due to bird strike of a turkey vulture near Fort Smith, Arkansas
10 Nov 1983 [ w/o] 82925 82- 0925 USAF 16 TFTS F-16A Block 15 K Details
Crashed on the Utah Test & Training Range killing Captain Christopher Barber. The unit was flying night mission. The pilot accidently perceived the flashing light on a train engine as the strobe of his flight lead and attempted to rejoin on the light with no radar lock.

Collided with an F-15A (75-0054) of the 122th FS over the Atlantic Ocean off Brunswick, Georgia. The pilot, Lieutenant Colonel John Michael Steward, was killed in this mid-air collision. The F-15 pilot ejected safely and was recovered.
12 Jun 1994 [ w/o] 82934 82- 0934 USAF 169 FS F-16A Block 15 K ADF Details
Written-off in a crash while making a fuel stop at Buckley ANGB, Colorado. When the aircraft lifted off the runway it veered off to the right with the pilot ejecting immediately and landing safe. Cause was determined as non proper trim settings applied for take-off.

F-16 CRASH

I couldn't find the Edwards AFB crash of 2001 in this list. I was trying to find what tail number it was.

Some time in the 80s a f16 landing at nellis missed the the arresting Hook and went thru the barrier at end of runway and came to rest in 3 pieces on the golf course

Pilot was cut out of cockpit and walked away
