First foreign F-16 loss. Fatal crash in Ijsselmeer, near Stavoren in the Netherlands. The aircraft was practising on the Vliehors range and testing the Orpheus pod. The pilot, Captain E.J.L. Kropman, was killed.

Aircraft was damaged beyond repair when it ran over the barrier at Leeuwarden AB, the Netherlands. The pilot, Lt. Filip Appeldorn, ejected safely and survived the accident. The mishap occurred during initial conversion training due to an engine problem. This was the first zero-zero ejection from an F-16

Crashed together with its sistership F-16A (78-0227, J-227) near Hoogeveen, Netherlands. The pilot, 2nd Lieutenant B. Van Winkel, did not survive the impact. Both aircraft were performing a close-in dogfight in dusk conditions and suffered what is known as Controlled Flight Into Terrain (CFIT).

Ran into trees after failing to clear a ridge during a low-level training flight in the Churchill River valley , 55nm southwest of Goose Bay. Structural damage to the wings and fuselage was the result. The aircraft recovered safely at Goose Bay. It is supposed to return to the Netherlands for instructional use. This F-16 was scheduled for retirement anyway

The aircraft skidded off the runway and landed on its left-hand side. The pilot ejected before the ditch, but the nose section was seriously damaged.

Crashed into the North Sea near Ameland, Netherlands. The pilot, Bas Pellemans ejected safely and was rescued.

Ran off the runway at Leeuwarden AB. The pilot, Captain J.C. De Vries, was unfortunately killed in the incident. The accident resulted from the ingestion of a heron into the engine at high speed during a heavy weight take-off roll. Since the hook system did not function correctly, the arrestor cable was missed and the aircraft left the end of the runway with a speed of approximately 100kts. Immediately thereafter it overturned and came across a ditch which tore off the nose gear and the nose section. When the aircraft came to a halt, everything ahead of the fuel tank was gone

Collided in mid-air with F-16A (84-1358, J-358) thirteen kilometres west of Grand Lake, Canada. The aircraft were flying out of Goosebay. The pilot of this aircraft, Capt. Rein "Snatch" Sneek, ejected safely

Ran off the runway at Leeuwarden AB. The two member crew survived. The aircraft is now on display at the Depot Mechanisch Vliegtuigmaterieel en Straalmotoren (Mechanical Aircraft Material and Jet Engine Depot) which is at Woensdrecht AB

On 9 sep 1986 F-16A J-622 suffered a landing mishap. The aircraft skid off the runway at high speed. Pilot Capt Jan “Jay Jay” Jeurninck ejected safely but the aircraft continued without him. The throttle still in idle position, fire fighters were having trouble getting the aircraft under control. To stop the engine they pumped halon into the intake. When they ran out of halon the engine revved up again but the short pauze was enough to get access to the throttle and stop it. The aircraft was returned to active duty with 311 squadron on 19 may 1987, it was sold and delivered to the Chilean Airforce on 3 jul 2007. There is a picture of this mishap.
