F-16 crash caused by bolt failure

A crash that destroyed a $19.2 million F-16 over the summer was caused by the failure of two quarter-inch bolts, Air Force inspectors said this week.
A U.S. Air Force F-16 pilot intentionally dropped an unarmed missile and two fuel tanks into a farmer's field Thursday after reporting a problem with his aircraft. The incident happened around 1400h.

G-LOC caused F-16CJ accident

Engine failure caused Cannon F-16 crash

Investigators have determined engine failure led to an F-16 crash on the Melrose Bombing Range in New Mexico March 21. The aircraft (#87-0330), assigned to the 27th Fighter Wing at Cannon Air Force Base, N.M., was destroyed. The pilot experienced minor injuries after ejecting from the aircraft.
United States Air Force investigators are trying to determine how a fin from a HARM fell from an F-16 and landed on a residential Florida neighbourhood.
Catastrophic engine failure caused the April 3 crash of a U.S. F-16DJ fighter near Misawa Air Base, Air Force investigators concluded.

A USAFE F-16CG from Aviano Air Base, Italy, crashes while deployed to Turkey for Operation Northern Watch. It was a non-combat mission. Pilot safely ejected.

Edwards F-16B crashes

The Edwards F-16B jet fighter aircraft which crashed this morning at about 7 a.m. was performing photo and safety chase support for another F-16 test aircraft.

The two jets took off from Edwards at approximately 6 a.m. to allow the test aircraft to perform a test of the Miniature Air-Launch Decoy, an air-launched missile that functions as a radar decoy, while the support aircraft provided photo and safety chase. Support aircraft routinely accompany test aircraft to obtain aerial photographic record of the mission and to visually monitor the mission?s safety in flight.
A U.S. Air Force F-16 pilot was killed late Tuesday when his plane crashed into a rice paddy in South Korea. The crash occurred about 10 p.m. as the pilot was on a training mission from Kunsan Air Base.

Cannon F-16 Accident Report Released

Investigators have determined engine failure led to an F-16 crash over the Gulf of Mexico Dec. 13. The aircraft, assigned to the 27th Fighter Wing at Cannon Air Force Base, N.M., was on temporary duty at Tyndall Air Force Base, Fla., at the time of the accident. The pilot experienced minor injuries after ejecting from the aircraft.

Danish F-16A crashes - pilot safe

On Monday April 2nd an F-16A from the RDAF crashed while doing ACM with three other F-16's. They all belonged to Skrydstrup AFB. The pilot, 36 years old Captain Allan O. Jensen, ejected safely and was taken to Aalborg AFB hospital in good condition.

Engine failure caused an Oklahoma Air National Guard F-16 to crash Oct. 12 near Atlanta, Kan. The aircraft was assigned to the 138th Fighter Wing at Tulsa International Airport. The pilot experienced only minor injuries after ejecting from the aircraft.

F-16 accident report released

Air Combat Command determined that a critical combination of avionics anomalies, procedural errors and individual mistakes - on the ground and in the air - led to the Nov. 16, 2000, midair collision between an F-16 and a Cessna 172 near Bradenton, Fla.
An Air Force fighter pilot committed a series of errors that "substantially contributed" to a fatal mid-air collision between an Air Force F-16 and a privately owned Cessna airplane over a Florida golf course last November, according to investigators.
