Air Force investigators have determined pilot error caused two F-16CGs to collide Dec. 18, while performing a four-ship night vision goggle training mission.
The two F-16CGs were recovered and both pilots were uninjured. The aircraft, assigned to the 4th Fighter Squadron, 388th Fighter Wing, Hill AFB, Utah, sustained combined damage estimated at $1,771,275.
Investigators determined the primary cause of the accident was the mishap pilot misprioritized his responsibilities. He focused his attention on analysis of a failed gauge and ceased performing the remaining procedures required to maintain his safe position in formation, resulting in a collision.
