- 21 Day Acknowledgement Letter/Spouse Statement of Understanding - signed by you and your spouse (if applicable), stating that they understand that job may require frequent TDYs, will not preclude financial restraints, and you and your dependents do not have any medical conditions.
- INFORMATION SHEET WITH EMAIL ADDRESS, DUTY PHONE
NUMBER, HOME PHONE NUMBER, HOME ADDRESS, COMMANDER’S NAME
AND DUTY NUMBER
Please include all of these items in a plain blue folder. There is no need to specialize your package. It will be broken down and tabbed before the routing process begins.
