Pilot Error, Lack Of Training Blamed For Helicopter Crash That Killed 12 Marines

A Marine investigation of a deadly 2016 helicopter collision off Oahu found that pilot error, lack of training and command problems were to blame.

The findings are contained in a report obtained by the Investigative Reporting Program at the University of California, Berkeley. The families of the 12 victims were recently notified of the Marines’ conclusions.

The two CH-53E Super Stallions, the oldest and largest Marine helicopter, were conducting a complex nighttime training mission, flying in formation, when the trailing aircraft slammed into the lead. Witnesses ashore heard and saw a fireball over the water, about one and a half miles from the town of Haleiwa.

The impact of the two Super Stallions resulted in a violent explosion with forces “estimated at hundreds of times the force of gravity” and “instantaneously” killed all aboard, according to the report.
