24 August 2011:
F-16 #6692 of the 401st Composite Wing, ROCAF, based at Hua Lien AB.

After 9 AM, during landing, flew too low, and left landing gear hit and broke lightning rod and metal frame on a power lines pole, 11 m AGL, at entrance of 12 Lane, Jia Bei 2 Street, Xin Cheng Town. Loud noise surprised residents.

Jia Xin Village mayor QIU Ming Xiong claimed fighters usually landed more to the north of the densely populated 12 Lane.

About 6-7 AM, two sorties of F-16s took off for regular training.
After 9 AM, F-16s RTB.
ROCAF explained they were landing on a combat-readiness runway that was seldom used.

31 July 2012: ROC MND confirmed media reports that flares bought for F-16s were defective. In each of 2005 and 2008, ROCAF bought MJU-7A/B flares.

Total quantity is 70 thousand shots.

ROCAF used and found some flares were defective.
In 2009, ROC MND sued supplier(s), and asked CSIST (Chung Shan Institute of Science and Technology) to resolve the problem. CSIST completed its research, and probably would improve the flares by 2013.

Reportedly, the two batches of flares cost over NT$ 60 mil. They can't, or fail to, fire. They remain in warehouses, and aren't used.

15 May 2013:
F-16A #6622 KE-450 of the 455th Composite Wing, ROCAF, based at Chia Yi AB.
  • About 14:00h, departed Chia Yi AB.
  • Around 14:30h, during one-aircraft training flight, crashed into sea, southwest of Chia Yi. Pilot ejected.
  • About 3:27 PM, rescued by ROCAF S-70C SAR helo, and returned to hospital at or near Chia Yi AB.

First Lieutenant WU Yan Ting
  • 26year
  • 90th Class, ROCAF Academy
  • Total flight time: +/- 300 hours
  • F-16 flight time: +/- 60 hours
  • Student/trainee pilot during conversion phase.

After this accident, ROCAF ORBAT has 144 F-16s.

5 October 2016:
F-16B. #6814.
455th Wing, ROCAF, based at Chia Yi AB.

10:49 AM, during landing, brake and landing gear malfunctioned, yawed away from centre-line of runway, and almost off the runway. Pilots managed to stop it at the west edge of runway, at 7,500' location.

During landing, left landing gear wheel was loose, and shook unnaturally.

Two pilots OK.

Chia Yi AP closed for three hours, until F-16 was removed from runway.

Civilian planes diverted to land at other airports.
Uni Air 8902, from Kin Men, was ETA 11:10 AM at Chia Yi, but landed at Kao Hsiung.

7:40 AM, took off, for training mission.

19 December 2016:
ROCAF F-16, based at Chia Yi AB, Chia Yi County.

Morning, took off, at Chia Yi AB, for training.
About 11:20 AM, one of two electricity generators malfunctioned.

About 11:28 AM, diverted to, and landed at CCK AB, Tai Chung County.
Aircraft and pilot(s) were OK.

Chinese article dated 2014/08/27, about a ROCAF group wedding ceremony on this date, at ROCAF Academy, Kao Hsiung.
Amongst 85 couples, 12 couples were husbands and wives who both were ROCAF soldiers.

Lin worked at Chia Yi AB.

10 August 2017:
F-16B ROCAF

While landing at Kang Shan AB, Kao Hsiung County, plane-spotters (duh) spotted two boxes of Zheng Ji rice cakes smuggled into the co-pilot's cockpit.

ROCAF confirmed the incident as returning from a training mission at Hua Lien AB, Hua Lien County, and cautioned a 401st Composite Wing Lieutenant Colonel and other pilots against unnecessary personal belongings in cockpits.

When asked by reporters, Zheng Ji the candy stores chain in Hua Lien City, thanked ROCAF for the endorsement (read: free advertising).
The store staff clarified the boxes should avoid direct sunlight, or the rice cakes would turn stiff and sour.

The concerned box contains rice cakes in four flavours: peanut, red bean, sesame, and yam; hand-made without preservatives.
Each box costs NT$ 225, or US$ 7.42.

Further, as August 14 is ROCAF Anniversary, that box when ordered on 2017/08/14 Monday before 23:59, price was NT$ 183, or 81.4% of original price.
Delivery would begin from 2017/08/16.

Reminds me of the scene in the movie Hot Shots! when protagonist inverts his fighter, and all the junk in the cockpit fall onto the canopy. 8D

Seriously, during the weekend (IIRC), ROCAF clarified pilots are allowed to have basic food and drink in cockpits, but dangerous items such as lighters should be carried in pods.

Leader of a flight of two.

Pilot was Major WU Yan Ting.
Wingman was probably Captain CHEN Zhi Yu.

(Initial news reports claimed accident airplane was F-16B with two pilots: Wu and Chen.
ROCAF clarified it was F-16A.)

By now, Wu had 1,039 flight hours total, and 736 hours in F-16.

Before 1:09 PM, took off, at Hua Lian AB.

1:43 PM, missing, near Nuan Nuan District, Keelung City (Ji Long City).
Wingman loitered, couldn't contact or find leader, and RTB by 2:40 PM.

If Wu ejected, ejection seat would send signal, but ROCAF didn't detect said signal.

Song Shan AP (Taipei City) scrambled at least one S-70C SAR helo, S/N C-493.

2:33 PM, Keelung City fire department began SAR.

ROC Coast Guard sent four ships to search the sea near Keelung City.
PP-3555, PP-3558, PP-10033, and PP-10065.

3:22 PM, New Tapei City fire department announced: a hiker reported he/she found airplane-like debris, at hiking trail 21 km point, on Wu Fen Mountain (Wu Fen Shan), Rui Fang District, New Taipei City.
Or, near weather radar station on Wu Fen Mountain.

After 4 PM, despite fog, on ground, Keelung City fire department personnel found burnt fragments of... biologic material, black box(es), documents, G-suit, parachutes, &c.

8 PM, ground SAR suspended until 6 AM, next dawn.

Back on Wednesday, 15 May 2013, then-First Lieutenant Wu ejected from ROCAF F-16A 6622.

2018/06/05: Wu's flight were aggressors tasked to attack Keelung Port.
According to flight plan, ingress from northeast, turn left, and egress to east.

Weather was cloudy / foggy, with low visibility.

2018/06/06: After all ROCAF F-16s were grounded for inspections, they resumed joint ROCAF-ROCN exercises above the sea near Tai Tung on 6 June. To uphold morale, 4th Composite Wing commander Major General CHEN Qi Qing co-flew a F-16.

Thursday, 21 June 2018
Morning, funeral ceremony, at Hua Lian AB. Attended by ROC President and ROC Minister of Defence.

Missing man formation by four F-16s, with #3 climbing away from the formation.

After ceremony, Wu's ashes were transported by C-130 to Taipei, then to Bi Tan Air Force Cemetery, Xin Dian, New Taipei City.

Monday, 2018/07/09:
ROCAF Command confirmed: today, about noon, flight data storage component of accident F-16 black box found and recovered. In publicity photo, NT$ 50 coin for size comparison.

Weeks ago, black box other components recovered were signal beacon, beacon battery, &c.

After accident, pilot's parents sued ROCAF Command.

On or about 2021/12/16, Taiwan Taipei District Court decided accident cause was five ROCAF personnel's fault, and awarded total NT$ 4,664,655 (about US$ 167,585) to pilot's parents. ROCAF Command could appeal.

Mission was simulated attack at Keelung Port, at low altitude. After attack, originally planned egress at heading 090, or east.

But that heading involved nuclear power plant no-fly zone, and controllers instructed pilot to change heading to 140, or southeast, towards Wu Fen Mountain. After pilot turned heading to 140, he requested permission to climb, two times.

19 seconds after he turned, controllers instructed pilot to climb. He began to climb, but still crashed into Wu Fen Mountain.

ROCAF personnel also neglected or violated the rule that aircraft must fly at least 2,000' above local terrain.

Looks like Taiwan just lost another F-16 which is an upgraded Block 72 V (Serial 6650).
