The aircraft was being towed from the Foxtrot taxiway (next to the alternative runway) coming from the washing area called a "birdbath" when the right wing hit some of the "birdbath" structure. The TSgt who was driving for some reason swerved the aircraft towards the birdbath instead of keeping it in the center of the taxiway. The structure of the birdbath stuck on the right wing pulling it off the taxiway. Most of the damage was the result of the nose gear collapsing in contact with the soft ground

The aircraft caught fire right after take off from Osan AB. The pilot, Maj. Kermis, circled around and was able to land. The core spray bar broke off and started spraying fuel in the enging bay. It was a class A mishap but the plane was repaired.

Returned from a training flight code one in Japan during a TDY. Taxiing back to it's parking spot the aircraft hit a pot hole causing a short in the ECM pod adapter. That resulted in sparks starting a fire. The aircraft was taken apart and shipped back in March 2005 to Mountain Home AFB and repaired by a team from Hill AFB. By July 2005 the aircraft was finally repaired and after a few test flights and scheduled upgrades was back in service. Mishap was graded a class A but was later downgraded to a class B. The aircraft was flown back to the United States aboard a C-5.

Left main tire blew after rotation during a take off from Alburquerque, New Mexico. Damage also was sustained when debris made a hole in the lower finger brace panel which is important to the the structure. It is thought to be repairable, but unknown if the wing will need to be replaced.

Crashed at the end of the runway at 8:34am while attemtping to land at Nellis AFB. The pilot ejected safely but was taken to the hospital to be treated for scratches. A briefcase got loose on the back-seat and jammed the throttle making it unable for the pilot to shut down the afterburner. The accident happened because the pilot failed to comply with emergency procedures.

Crashed into a marshland area next to Ashley River near Charleston, South Carolina at about 5 pm local time. The aircraft was from the 55th FS but the crew were from the 77th FS. Both of them ejected safely and walked away but were examined at hospital as a precaution. The aircraft was on a routine training mission when it lost engine power.

Came in to low for landing and struck some runway lights which caused the main landing gear to collapse. Colonel Douglas Fick stayed with the aircraft and was uninjured. The landing happened in severe weather conditions which made the pilot to abort two consequent landing attempts. Trying for a third attempt, he came in too low.

The pilot tried to land the plane at Lamar Airport after some fire warnings went off in the cockpit. When the pilot, Julian Clay, realized he wouldn't be able to stop before hitting the end of the runway, he ejected, only suffering minor injuries. After the ejection the plane ran off the runway and split in two. This was the second ejection of this pilot, already ejecting from #86-0325.

Damaged in flight during an air refueling with a KC-10A #83-0081 at 10:31 hours. The aircraft landed safely but it is estimated to be some time to be repaired as a bulkhead was severely damaged. Closure rate of the F-16 to the KC-10 was slightly higher then normal. The boom operator pulled the boom up too quickly and in an effort to prevent the boom from hitting the tail of the KC-10 the boom operator overcompensated and the boom came back down hitting the F-16. This was barely a class A mishap.
