The aircraft crashed at 11:29 am at about 1.6 miles northeast of the town of Tanosawa, Japan. The pilot, Maj. Chad Miller, ejected from the aircraft with minor injuries. Approximately 15 minutes after beginning a qualification training upgrade mission, Miller reported an engine problem. He immediately turned toward land and attempted to restart the engine four times without success. He turned the aircraft parallel to the coast away from populated areas and then ejected. The aircraft was destroyed upon impact with the water. Based on evidence obtained during the investigation, the accident investigation board president's opinion is that a fatigue crack had developed in a high-pressure turbine blade. The crack caused a portion of the blade to fall off that then caused catastrophic damage and failure of the remaining turbine blades. Once the turbines failed, the engine could no longer produce thrust, continue to operate or be restarted.

Impacted the ground near Bobby Chain Airport in Hattiesburg, Mississippi. Pilot, Major Charles Toplikar, ejected safely with minor injuries. The primary cause of the accident was the failure of the high-pressure turbine post, allowing the HPT blades to break free and damage the engine. Unit had just passed 50000 hours mishap free when this happened.

Starboard main gear tension bar failed after being loaded with fuel and weapons at Decimomannu. The aircraft was parked and chocked at the time. It was marked ‘16 AF’ and was on detachment at the time. Cause was a corrosion on landing gear.

Destroyed in a mid-air collision with F-16C (#89-2111). Capt. David Rosmann, ejected safely. The mishap was caused by the failure of both pilots to properly deconflict their flight paths during a tactical turn. Other contributing factors included: loss of situational awareness, misinterpretation of closure and visual cues, task misprioritization and channelization, and expectancy.

While flying over Utah salt flats, pilot was not aware of how close he was to the ground as there was a layer of very calm water, which made a mirror like image making it difficult to judge distance to the ground. Pilot, Lieutenant Colonel Dillon McFarland, was killed.

Arizona ANG almost lost a two seater on this day when it belly landed at Tucson IAP on its centerline tank which caught fire but did not burn the aircraft. This aircraft was repaired and put back into service. Damage was mostly done to the centerline tank, left stab and a missile. The pilots, Lt. Col. Kent Dukes and Italian backseater, Capt. Salvatore Trincone, were uninjured.

Mid-air collission with F-16C (89-2072). Both aircraft and their pilots returned safely to Hill AB. There was $ 1.7 million worth of damage to both aircraft.

06 Feb 2003 [act] 90718 90-0718 USAF 18 FS F-16C Block 40H News Article Details
Became loose during a ground engine test at Eielson AFB, Alaska and travelled across the ramp 400 feet. Aircraft ended up in a balancing act on its nose and starboard wing on the ground and port landing gear in the air. The aircraft will be fixed although there was some extensive damage. A ground crew personnel inside the aircraft was fine. It was a hot oil servicing run that was being conducted and the plane was being run on hard packed snow. The plane jumped chocks and slid sideways.
29 May 2003 [w/o] 89052 89-2052 USAF 36 FS F-16C Block 40E News Article Details
Crashed at the end of the runway after pilot ejected safely during takeoff at 20:06 hours at Osan AB, Korea. Two ducks were sucked into the intake and caused the crash. A woman driving her car just inside the main gate was injured as the debris crossed her path. Large portions of wreckage crossed the road inside the Doolittle gate and into an aircraft historic park.
10 Jun 2003 [w/o] 88451 88-0451 USAF 310 FS F-16C Block 42C News Article Details
Lost at 16:50 hours at Barry M. Goldwater training range in Arizona. Captain David O’Malley ejected safely. It was suspected that it was a 4th Stage turbine failure from a manufacturing defect. All F-16's of this engine type were grounded as a result of this loss. A fault in the engine was found and remaining aircraft began flying once this part was replaced. Work was expected to be completed by March 2005. It is now required to inspect these fan blades periodically.
