Written off at Wright-Patterson AFB for unknown reasons. It was then used for Aircraft Battle Damage Repair Training (BDRT), even after the squadron exchanged the type for C-141's.
24 May 1993 [w/o] 90832 90-0832 USAF  F-16C Block 50B Details
First pre-delivery crash (company acceptance flight). The pilot attempted a split-S at 2,100 feet and was forced to eject at only 680 ft with his nose 40° low. The pilot (LMTAS senior experimental test pilot), Joe Bill Dryden, was killed. The aircraft crashed 20 miles west of Fort Worth near Mineral Wells, Texas. Bill was unable to separate from the ejection seat before impacting

Mid-air collision with F-16B (82-1042) during a training sortie. The aircraft landed safely.
23 Jun 1993 [w/o] 82042 82-1042 USAF 169 FS F-16B Block 15N ADF Details
Crashed at Peoria, Illinois due to a mid-air collision with F-16A (82-0916) during a training sortie. The F-16A came up from below and impacted the F-16B. The crew ejected with only minor injuries while the F-16A landed safely
Some sources have this aircraft flying with the 170th FS at that time, but it was the 169th FS.
27 Jul 1993 [w/o] 86275 86-0275 USAF 35 FS F-16C Block 30D Details
In poor weather this F-16 landed on top of F-16C (87-0335) which was trying to take off . Pilot Lt. Col. Ralph Earl "Bark" Gardner who had just arrived at Kunsan AB, Korea from MacDill AFB was killed. The 56th FW which was closing down had just enough aircraft to fly for his memorial.

Was preparing for take off when F-16C (86-0275) landed on top of it. One of the pilots, Lt. Col. Ralph Earl "Bark" Gardner who had just arrived at Kunsan AB, Korea from MacDill AFB was killed. The 56th FW which was closing down had just enough aircraft to fly for his memorial.

Wore the Su-27 colour scheme and numbered '50' at the time of the mishap. The pilot, Captain James Reynolds, did not survive this crash.

Flew into the Adriatic Sea. The pilot had ejected safely

Crashed at the Cape May County Airport, New Jersey (Vermont?). The aircraft was owned by the Vermont ANG
11 Sep 1993 [w/o] 81779 81-0779 USAF 169 FS F-16A Block 15G ADF Details
Belonged to the Illinois ANG. The aircraft was completely destroyed in a crash. It impacted the desert in the Nellis range, Nevada
27 Sep 1993 [w/o] 86253 86-0253 USAF 302 FS F-16C Block 32C Details
Collided at Gila Bend, Arizona with another F-16 which landed safely.
08 Oct 1993 [w/o] 91350 91-0350 USAF 23 FS F-16C Block 50C Details
The aircraft crashed at 1830 hours on a delivery flight at Remsen, near Griffiss AFB, New York. Air Force investigators have concluded that night fog and a blown fuse in instruments that receive landing information were the cause of the crash. Pilot ejected safely. The F-16 cut a 1,000-foot swath through the trees off Joslyn Road before coming to rest in a fiery heap 500 feet from the home of Nick and Diane Polce. The aircraft was being delivered from Fort Worth, Texas to the 23rd FS in Spangdahlem AB, Germany.
08 Nov 1993 [w/o] 88448 88-0448 USAF 57 FW F-16C Block 42C Details
The pilot, Captain John M. Barelka, died in this crash.
09 Nov 1993 [w/o] 86325 86-0325 USAF 120 FS F-16C Block 30E Details
Went down near Laramie, Wyoming. The pilot was Lt. Julian Clay. Probable cause was an engine failure.
29 Nov 1993 [w/o] 81770 81-0770 USAF 114 FS F-16A Block 15G ADF Details
Lost near Tulelake, California. The pilot, 1st Lieutenant Stephen Taylor, a student pilot from the Vermont ANG, died in the crash
