Crashed at Carswell AFB, Texas while doing a mock airfield attack. Three B-52's on the ground were slightly damaged. Both crew were killed. The front seat pilot was Captain Rob 'Pork' Tucker. The pilot in the back seat of the plane, Captain J. Bourne, was a pilot from VMFA-112. He was on an incentive flight, his first time to fly in a F-16. After doing a low pass over the runway at 350 to 400 knots, they did a 90 degree bank. This in turn was a very high "G" maneuver in which they slammed into the ground . The investigation determined G-lock. The plane disintegrated on impact between the runway and taxiway.

Engine turbine failed during a take-off roll sending pieces of the turbine blades through the fuel cell. Pilot aborted take off after seeing the fire light and shut down the engine. After the pilot opened the canopy and jumped out found the entire tail of the F-16 on fire. The aircraft was sent to depot but was never repaired.

Belly landed but stayed on the runway. Pilot did not eject. Aircraft carried wing commanders markings. Minor repairs were made but the aircraft never flew again and went to Sheppard AFB, Texas. Cause of the mishap happened as follows. The centerline tank was installed prior to flight. Leak and transfer check where good, but the pilot did not close the IFR door. The pilot noticed fuel not transferring and punched off the full centerline tank. The F-16 came into view for landing powered by the EPU only and kept the gear up. When the aircraft touched down bright colored flames came from under the aircraft as it slid to a stop, but it did not catch fire.

Was restored to flyable status but suffered a blow out of the tail section due to a fire during a Cope Thunder exercise. The pilot dead-stick landed it. It took 8 months to repair. Rumors were the aft section of a previously damaged block 40 was used to replace the tail of this aircraft. It was later unofficially labeled a block 35. Any one know the exact date of this mishap?

Aircraft had a leading edge malfuntion during flight which ripped off the majority of edge. It landed safely and was repaired. Need to find the date of this mishap.

Crashed near Homestead AFB, Florida. The aircraft was lost due to a burn-through in the engine and airframe. The pilot ejected when ATC personal saw a fire plume coming from the side of the aircraft. It was later determined that during a flame holder assembly change, a maintenance rag was left in the afterburner section causing a hot spot. The airforce F-16 units then began counting rags taken out on a daily basis. The aircraft was viewed with the vertical stabilizer above the water as it crashed in shallow water at the end of the runway.
