Crashed near Fall River, Kansas. Student was on basic fighter maneuvers and exceeded limiters and F-16 lost control. The pilot ejected and survived.
16 Aug 1989 [w/o] 82954 82-0954 USAF 309 TFS F-16A Block 15L Details
Lightning struck the aircraft near the centerline tank. The resulting fire forced the pilot to eject near Homestead AFB, Florida just short of the runway.
14 Sep 1989 [w/o] 81671 81-0671 USAF 186 FIS F-16A Block 15C Details
Impacted the ground in Arizona near the boarder of New Mexico. The aircraft was on temporary alert detachment at Davis Monthan AFB, Arizona. Pilot ejected safely.
15 Oct 1989 [w/o] 87369 87-0369 USAF 302 TFS F-16D Block 32F Details
Crashed at Carswell AFB, Texas while doing a mock airfield attack. Three B-52's on the ground were slightly damaged. Both crew were killed. The front seat pilot was Captain Rob 'Pork' Tucker. The pilot in the back seat of the plane, Captain J. Bourne, was a pilot from VMFA-112. He was on an incentive flight, his first time to fly in a F-16. After doing a low pass over the runway at 350 to 400 knots, they did a 90 degree bank. This in turn was a very high "G" maneuver in which they slammed into the ground . The investigation determined G-lock. The plane disintegrated on impact between the runway and taxiway.
Nov 1989 [i/a] 78002 78-0002 USAF 310 TFTS F-16A Block 1 Details
Engine turbine failed during a take-off roll sending pieces of the turbine blades through the fuel cell. Pilot aborted take off after seeing the fire light and shut down the engine. After the pilot opened the canopy and jumped out found the entire tail of the F-16 on fire. The aircraft was sent to depot but was never repaired.

Destroyed near Homestead AFB, Florida
18 Dec 1989 [w/o] 84263 84-1263 USAF 496 TFS F-16C Block 25D Details
Collided with F-16C (#84-1293) over Maxdorf, Germany. One pilot, First Lieutenant Steve 'Sunny' Sundstrom died, the other pilot, Major Rodd Kallman ejected safely.

Collided with F-16C (#84-1263) over Maxdorf, Germany. One pilot, First Lieutenant Steve 'Sunny' Sundstrom died, the other pilot, Major Rodd Kallman ejected safely.
19 Dec 1989 [i/a] 87251 87-0251 USAF 36 TFS F-16C Block 30F Details
Belly landed but stayed on the runway. Pilot did not eject. Aircraft carried wing commanders markings. Minor repairs were made but the aircraft never flew again and went to Sheppard AFB, Texas. Cause of the mishap happened as follows. The centerline tank was installed prior to flight. Leak and transfer check where good, but the pilot did not close the IFR door. The pilot noticed fuel not transferring and punched off the full centerline tank. The F-16 came into view for landing powered by the EPU only and kept the gear up. When the aircraft touched down bright colored flames came from under the aircraft as it slid to a stop, but it did not catch fire.

Destroyed near Kunsan AB, South Korea.
Unknown [act] 85557 85-1557 USAF 14 TFS F-16C Block 30B Details
Was restored to flyable status but suffered a blow out of the tail section due to a fire during a Cope Thunder exercise. The pilot dead-stick landed it. It took 8 months to repair. Rumors were the aft section of a previously damaged block 40 was used to replace the tail of this aircraft. It was later unofficially labeled a block 35. Any one know the exact date of this mishap?
Jan 1990 [act] 84292 84-1292 USAF 313 TFS F-16C Block 25E Details
Aircraft had a leading edge malfuntion during flight which ripped off the majority of edge. It landed safely and was repaired. Need to find the date of this mishap.
15 Jan 1990 [w/o] 83089 83-1089 USAF 308 TFS F-16A Block 15R Details
Crashed near Homestead AFB, Florida. The aircraft was lost due to a burn-through in the engine and airframe. The pilot ejected when ATC personal saw a fire plume coming from the side of the aircraft. It was later determined that during a flame holder assembly change, a maintenance rag was left in the afterburner section causing a hot spot. The airforce F-16 units then began counting rags taken out on a daily basis. The aircraft was viewed with the vertical stabilizer above the water as it crashed in shallow water at the end of the runway.
