Crashed near Goose Bay, Newfoundland. The pilot, Captain Bob Verkroost, was able to eject safely. The pilot was on a low level training flight. Captain Verkroost later ejected from J-359 on September 23, 1992.

This mishap happened somewhere in 1989. Pilot L. van Bruggen executed a touch-and-go but came back on the runway with a retracted gear.

Crashed near Javinque, Belgium due to an engine failure. The pilot ejected safely.

Collided in mid-air with F-16A (84-1358, J-358) thirteen kilometres west of Grand Lake, Canada. The aircraft were flying out of Goosebay. The pilot of this aircraft, Capt. Rein "Snatch" Sneek, ejected safely

Collided in mid-air with F-16A (78-0258, J-258) thirteen kilometres west of Grand Lake, Canada. The aircraft were flying out of Goosebay. The pilot of this aircraft, Capt. Leo "Moist" Vogt, was killed in the incident

Crashed in Coevorden which is near Dalerpeel, Netherlands. The pilot ejected safely. The loss was attributed to a flight control problem
28 Feb 1991 [w/o] J-200 83-1200 RNLAF 323 sqn F-16A Block 15U Details
The aircraft went down in the North Sea about 130 nautical miles north west of Texel, Netherlands. The aircraft was on a sortie from RAF Coningsby. The pilot, 1st Lieutenant Ed W. Gelens, was killed in the incident.
21 May 1991 [w/o] J-880 81-0880 RNLAF 312 sqn F-16A Block 15T Details
Control was lost in a dive after an unrecoverable engine stall during air combat manoeuvres on a sortie from Decimomannu. The incident took place over the Mediterranean, 18 miles off the western coast of Sardinia, Italy. The pilot ejected safely.
10 Jan 1992 [w/o] J-007 88-0007 RNLAF 313 sqn F-16A Block 15AG OCU Details
Crashed on the Vliehors range in the Netherlands. The pilot, 1st Lieutenant Willem van Suydam, did not eject and was killed in the crash. It is believed to have been target fixation. This happened near Vliehors range, Frisian Islands
11 Feb 1992 [w/o] J-054 86-0054 RNLAF 315 sqn F-16A Block 15Y OCU Details
Crashed into an occupied area of Hengelo, Netherlands. Fortunately there were no injures and the pilot ejected safely. Shortly after takeoff, there were engine problems which resulted in the crash.
26 May 1992 [w/o] J-618 80-3618 RNLAF 311 sqn F-16A Block 15 Details
Crashed during a training sortie 120 miles south of Goose Bay, Newfoundland. The pilot, Capt. Jac Jansen, ejected safely from the aircraft which was experiencing engine problems
23 Sep 1992 [w/o] J-359 84-1359 RNLAF 323 sqn F-16A Block 15W Details
Crashed in a residential area in the small town of Arum, Netherlands due to an unrecoverable engine stall. The pilot ejected safely, but a civilian on the ground was injured. The pilot was Captain Bob Verkroost who also ejected from F-16A J-625 on June 4th, 1988. This probably makes him the only Dutch pilot to eject twice from an F-16.
18 Dec 1992 [pre] J-238 78-0238 RNLAF 322 sqn F-16A Block 10 Details
Departed the runway during a landing. The aircraft rolled over injuring the pilot. He was attempting to land at Leeuwarden AB
10 Feb 1993 [w/o] J-195 83-1195 RNLAF 311 sqn F-16A Block 15T Details
Flight control problems caused the loss. The crash occurred near Geffen in the Netherlands. The pilot, "Torch" Timmers, ejected safely.
