During a functional check flight the aircraft experiences an afterburner stall. The pilot reduced power, but the temperature continued to rise. The pilot had to stop the engine and restart the engine. He made a simulated flame-out landing at Kleine-Brogel AB. The pilot was Cdt. Rudy 'Chouke' Schoukens

Nose wheel separated during landing, but is expected to be repaired. The pilot was fine

Less than a month after delivery, the aircraft flies through some high voltage power cables in the valley of the Dordogne, Southern France. The pilot, 2nd squadron commander Cdt. Alain Letellier, succeeds in landing at Aulnat (near Clermont-Ferrand). The aircraft was heavily damaged on the belly, but was repaired

Collided with F-16A (80-3584, FA-93) at 11:30 hours near the towns of Somal and Failon, Belgium. Both aircraft were conducting a FAC exercise at the time of the accident. One pilot, Lieutenant Colonel Ronny Vaerten (FA-93) didn't eject and was found in the wrecked cockpit while the other pilot, Lieutenant Wouter Rombouts ejected, suffering minor injuries.

Suffered an engine flame-out. The pilot, Maj. Frank Dewinne, succeeds in an airstart and landed safely at Leeuwarden AB, Netherlands

Engine bird strike at Schleswig-Jagel AB, Germany. The aircraft was performing a fly-by for the 2004 Tigermeet. Captain Steven 'Vrieske' De Vries landed the aircraft safely.
07 Mar 1997 [act] FA-127 89-0005 BAF 2 W F-16A Block 15 OCU Details
Minor mishap involving this F-16A. Nose wheel collapsed after landing. Damage was minor and only to the nose gear door. The pilot stayed with the aircraft since it was not moving fast during the incident. The nose gear collapse took place at Florennes. The aircraft was repaired

Dropped an ALQ-131 ECM system while on a training flight. No-one got hurt, but there was some damage to parked cars near a retirement home.
16 Oct 1997 [act] FA-134 90-0025 F-16A Block 15 OCU Details
Caught fire during take-off from Villafranca AB, Italy. It was repaired only a week thereafter.
