Can't let this happen again. Suggest TCTO 2012 ... High intensity LED Brake light install on horizontal stab static discharger tips, and TCTO 2013 ... Software update to GCAS = GGCAS 1.1 (Ground to Ground Collision Avoidance System!!!

You know what really happened right?

AIB ... "The accident investigation board found clear and convincing evidence the cause of the mishap was the pilot's failure to properly monitor his aircraft's position relative to the aircraft in front of him, due to a breakdown in visual scan, task mis-prioritization, and channelized attention."

Varkvet... Since he was #4 he was "Texting while Taxiing" ... he was was a rookie and was trying to finish putting the briefed info in the UFC while taxiing.

Normally you see the junior pilots show up and start well in advance of the Pro's, just to make sure they are ready to go when when the leaders are?

didnt they also suggest an “excessive motivation to succeed” as an additional cause?

I think that qualifies as as BSing a report. Excessive motivation to succeed...in what? Excuse me if I'm missing anything, but if anything it seems he was unmotivated and distracted.
