What is this unique backbone I've seen on Greek and Chilean Block 50/52 Vipers?

It's not unique. First time seen in Israel F-16s. Because the 2-seat models (D,F) don't have any space left for internal jammer, they were suited with this "backbone". It provides more space for electronics (jammer) and countermeasures (chaff, flares) but NOT additional fuel.

I've never seen it on any U.S. 2 seat models or CFTs. Are they export only?

The back bone for extra equipment is part of the block 50/52+ and the block 60, which the USAF did not buy any of. That is why you have never seen a USAF F-16D model with that back bone.

Also because there isn't a tactical need for them. USAF uses only external jammers in pods. The backbone provides more space for the jammer to the 2-seat versions (missionised aircrafts).

Hi,

One USAF F-16D Block 30 flies with this high back configuration also. It is the one-off NF-16D 86-0048 VISTA/MATV demonstrator once belonging to the Calspan Corp. BTW, all Israeli F-16D Block 30/40 aircraft already had this feature.

Not exactly. Israeli Block 30 and 40 D models are equipped with the enlarged spine as well.
