During an air raid over Syria the aircraft was locked on by Syrian missile defence systems. They fired over 10 missiles at the aircraft with one of them hitting the F-16. It crashed within Israeli territory near Harduf with both pilots ejecting. They landed in Israel with one of them being critically injured.

During an exercise the aircraft hit a power line. It was damaged, but able to land safely.

Crashed during a training flight in the early evening. Cause is still under investigation. The pilot was killed in the mishap.
04 Apr 2018 [ w/o] 91413 91- 0413 USAF USAF ADS F-16C Block 52 D News Article Details
Crashed at the Nevada Test & Training Range at 10:30am local time, killing the pilot, Maj. Stephen Del Bagno. Accident was caused by temporarily loss of consciousness during a high G-force maneuver.
24 Apr 2018 [ w/o] 90760 90- 0760 USAF 310 FS F-16C Block 42 J Details
This F-16 departed the runway at Lake Havasu City Municipal Airport on 10:35am local time. The F-16 diverted to the airport during a routine training mission. The aircraft landed on runway 14 but departed the runway for unknown reasons. The pilot ejected safely. The aircraft suffered extensive damage with the cockpit section torn off and substantial damage to the rest of the fuselage. It is considered a write-off.

Damaged when deployed to Al Jaber AB. A heavy sandstorm with 91-mile per hour winds overturned some sunshelter structures who came down on the aircraft.

04 Jun 2018 [ w/o] 6685 93- 0786 RoCAF 27 TFG F-16A Block 20 Details
The aircraft performed a simulated attack at low altitude at Keelung Port. The original planned egress from the attack was at heading 090. However that heading involved a nuclear power plant no-fly zone and controllers instructed the pilot to change heading to 140 towards Wu Fen Mountain. After the pilot turned heading to 140 he requested permission to climb, two times. Nineteen seconds after he turned, controllers instructed pilot to climb. He began to climb, but unfortunately still crashed into Wu Fen Mountain at 13:43 local. The pilot, Major Wu Yen-ting, was killed in the mishap.

The aircraft suffered a devastating ground firing after accidentally being fired upon from another F-16 at Florennes AB around 14:10 hours local time. A second aircraft sustained some minor damage and two men sustained some minor injuries.

Several bullets fired from another parked F-16 struck the vertical tail, fuselage and engine. The aircraft next to this one took the brunt of the destruction (FA-128) and burned up. This aircraft will be repaired.
30 Oct 2018 [ sto] 493 00- 1047 IDFAF 119 sqn F-16D Block 52 Details
Ran off a taxiway and hit a lighting pole at Ramon Air Base, Israel. Although the landing gear did not collapse, it did tip up on it's nose and suffered some minor damage, perhaps more if the motor ingested too much debris. Crew were able exit the aircraft on their own. Exact date of loss is not known.
09 Nov 2018 [ sto] 96084 96- 0084 USAF 55 FS F-16C Block 50 Details
The aircraft sufered a mishap during take-off in which it hydroplaned off the runway at Shaw AFB. The pilot was unhurt, but the aircraft received extensive damage.
