The aircraft slid off the runway at Ogden airport after being diverted there from Hill AFB. The aircraft suffered a malfunction in the nose landing gear. The pilot stayed with the aircraft and exited safely. The targeting pod was damaged and the engine will be replaced and repaired.

The aircraft crashed during a training flight 5 miles east of the city of Ioannina at 10:33h local time. The pilot ejected safely.
22 Jun 2009 [ w/o] 89108 89- 2108 USAF 421 FS F-16C Block 40 G News Article Details
The aircraft crashed in a remote part of the Utah Test And Training Range at 22:25h local time. The pilot, Captain George Bryan Houghton, was killed in the mishap. The crash was caused by target fixation and disorientation due to low night illumination.

The plane was on a routine night training mission when it crashed close to village of NurPur, 105km south west of Sargodha. The pilot, Squadron Leader Saud Ghulam Nabi, was sadly killed.

During a night training sortie #91-0364 collided with #91-0365 over the Atlantic Ocean approx 126 nautical miles southeast of Shaw AFB. The aircraft suffered moderate damage to flight control surfaces and external stores. The jet recovered to Charleston AFB. #91-0365 crashed after the impact and was destroyed.

Crashed at 13:45 hours local near the village of Bnei Haver. Dove into the ground after executing ACMI training. The pilot, Lieutenant Assaf Ramon (son of Col. Ilan Ramon), was killed in the accident. Cause of the accident was determined as G-LOC.

Damaged on landing at Korat AB where it engaged the barrier at the end of the runway. One landing gear was broken and both wing leading edges were damaged.
Oct 2009 [ act] 88440 88- 0440 USAF 36 FS F-16C Block 40 B Details
Was hit on the rudder by a taxiing KC-135. The aircrew of the KC-135 didn't not follow commands of the crew chief at EOR and continued to taxi by, hitting the rudder on the middle static discharger. The aircraft was quickly fixed.

During a night training sortie #91-0365 collided with #91-0364 over the Atlantic Ocean approximately 126 nautical miles southeast of Shaw AFB. The pilot did not survive the collision
22 Oct 2009 [ i/a] 84328 84- 1328 USAF 62 FS F-16D Block 25 E News Article Details
Made an emergency landing short of the runway at Wickenburg municipal airport at approximately 10:30h after losing engine power. Both crew exited the aircraft safely. Could be repaired, but since it's an older block 25 airframe it probably won't be fixed.
