The aircraft crashed into the Gulf of Mexico west of Key West at about 19:30 hours local. The pilot, Major Peter S. Smith, ejected from the aircraft and was picked up by a Navy helicopter. The cause of the accident was spatial disorientation.

Crashed just outside the base perimeter at Monte Real AB during its first test flight after completing M4 MLU modification. Wreckage came down in a pine tree plantation at 13:40 hours causing a small fire. The pilot, Lt.Col. João 'Skipper' Pereira, ejected safely and no-one on the ground was injured.

Crashed into the sea at 20:00h off the coast of Sicily in between the islands of Levanzo & Marettimo during a night training exercise. The pilot, commander Maurizio De Angelis, ejected safely sustaining limited injuries.

Crashed into the sea after losing contact with air traffic controllers at 19:18 hours. A SAR operation is underway to find the pilot, Major Ting Shih-Pao, but no results so far.
14 Mar 2008 [ w/o] 84273 84- 1273 USAF 62 FS F-16C Block 25 D News Article Details
Lost at 12.17h local time just south of Alamo Lake, Arizona. The aircraft was on a two ship air-to-air student training mission at the time. Sadly the pilot, Second Lieutenant David J. Mitchell of the 112th FS did not survive. He had 237 flight hours of which 26 were in the Viper. Mitchell would have graduated Initial Pilot Training in June of 2008.

Overran the runway upon landing. Damage was minimal.

Belly landed at Gila Bend Auxiliary Field, Arizona while returning from a training mission. Pilots were one instructor pilot and one pilot training to become an instructor pilot. As a precaution, both pilots were taken to Luke AFB medical facility.

Damaged during a shooting exercise at Hill AFB. 20mm rounds from the cannon bursted near the cockpit section and consequently damaged it. The aircraft will be repaired at Ogden Logistics Center.

Lost during a routine training mission sadly killing the pilot Hamzah Samir Odeh Al-Abbadi. The crash occured at 13:10 hours.
21 Apr 2008 [ w/o] 9533 85- 1542 EAF 242 TFW F-16C Block 32 B Details
Reportedly crashed about 80 miles south of Beni Suef AB, Egypt killing the pilot.
29 Apr 2008 [ act] J-617 80- 3617 RNlAF 313 sqn F-16A Block 20 MLU Details
While taxiing at night during on a deployment to Cold Lake, Alberta the aircraft departed a taxiway and the nose gear sunk into the ground shearing it off. The aircraft is repairable.
15 Jun 2008 [ act] J-144 85- 0144 RNlAF 323 sqn F-16A Block 20 MLU News Article Details
Damaged in an aborted take-off from Kandahar airfield, Afghanistan for an ISAF mission. The pilot stayed with the aircraft which engaged the barrier at the end of the runway. The pilot was fine.
12 Nov 2008 [ w/o] 93554 93- 0554 USAF 55 FS F-16C Block 50 Q News Article Details
Destroyed on the ground when a fire broke out while the engine was running at Balad AB, Iraq. Pilot opened the canopy and escaped without any issues. Fire destroyed a lot of the fuselage and warped the right wing. Investigation and details pending.

Overshot the runway at Florennes AB during a TLP exercise. The pilot ejected safely. The aircraft was only slightly damaged and is deemed repairable.
