During an engine check before take-off, a malfunction caused engine to engage full afterburner. The aircraft ran off the ramp and burned after sinking into a ditch and striking a tree. Location was Nea Ankhialos. The aircraft was taken to Hill AFB, Utah where it was repaired. It was flown again on October 12, 2001 by 514 Flight Test Squadron's Major Jeffrey Clayton. This Greek aircraft spent 4 years at Hill AFB being repaired and is the most severely damaged F-16 to be repaired. Some parts had to be retooled as the block is older and some parts no longer made.

Went down 2 miles southeast of Strum, Wisconsin at 10:20 hours after engine began to loose thrust. The pilot, Captain John C. Wasserburger, ejected safely before aircraft impacted the terrain. The F-16 belonged to the Wisconsin ANG at the time of the crash.

Crashed after colliding with F-16D (90-0895, 660) near Nehariyyal, Israel. The pilots escaped and were pick-up from the sea.

Crashed after colliding with F-16D (90-0877, 606) near Nehariyyal, Israel. The pilot, Lt. Col. Roni Shlomi and his navigator, Captain Yuval Binyamin, were both killed.

The airplane crashed near Parks, Arkansas with the pilot - Dean Fontenot - ejecting safely. The cause of the crash was an engine failure.

Crashed 100 miles northwest of Luke AFB near Prescott, Arizona at 16:58 hours. The pilot, Captain Nathan Hill, ejected safely from his aircraft which was on a routine training mission. It suffered an engine failure.

Crashed near Akinci AB due to a gun cartridge entering the jet intake during a high G strafing maneuver.

The aircraft made a belly landing with extensive damage to the underside and internal cables. Lockheed seemed the aircraft unrepairable (at least economically), but the TuAF decides to repair it anyway.

Crashed due to an engine failure at Yedigoller (seven lakes) region near Bolu. The pilot, 1st Lt. Hakan Dinc, ejected safely only suffering minor injuries.

Engine cut while returning to its base at Nea Angchialos in company with another F-16. The engine cut at 8,000 ft, but after unsuccessfully starting the engine, the pilot ejected safely at about 1,000 to 1,400 ft above the sea just north of Skiros Island

Crashed while performing an aerobatic display over the base at Maracay/Palo Negro for an air show. Both of the crew were killed. They were the first and second commanders of the squadron

Crashed near the Dugway Proving Grounds, some 130 miles southwest of Hill AFB, Utah. The pilot, Captain Christopher Rappa, who was on a training mission ejected safely. The aircraft had just been transfered to the 4th FS from the 421st FS a few weeks earlier and new paint scheme applied. Cause of crash was simply a stall at 14,000 feet and aircraft pancaked onto the ground upside down. With no forward volocity at the time of impact, the crash site was little larger then the F-16 itself.
21 Dec 1995 [ w/o] 84250 84- 1250 USAF 62 FS F-16C Block 25 D Details
The F-16 crashed at 10:20 hours after engine failure. The pilot, Lieutenant Colonel Joseph A. Abbott, ejected safely. The mishap took place on routine training mission about 12 miles southwest of Winslow, Arizona. Abbott was on temporary duty from Cannon AFB, New Mexico to Luke AFB.
