Crashed onto the Utah Test & Training Range killing pilot, First Lieutenant S. Brad Peale. The aircraft suffered a controlled flight into terrain (CFIT).

Crashed at Havrenne (near Rochefort), Belgium after Captain Etienne De Meulenaer ejected safely

Ground crew were conducting an engine trim at night when a 18 lb. piece of the N1 compressor liberated itself by way of the No 1 fuel cell. The plane exploded. Everyone survived but two were treated in Izmir, Turkey and released and one was evacuated to Wiesbaden, Germany and remained in the burn clinic for close to 30 days.

Crashed into the Mediterranean Sea, 22 miles from Adana, Turkey. The pilot, Captain Glen E. Dupuis, was killed in the mishap. The aircraft was on a DACT with F-15's from BT. The pilot experienced spatial orientation issues and hit the water at Mach 1+.

Collided with F-16B (82-1029) over the Nellis Range, Nevada. Captain Jerry Noble Allison was an A-10 pilot on an incentive ride in the F-16B back seat and was killed.
