Pilot received a warning light indicating a failure of the nose gear to lock. Upon landing the pilot kept the nose up as long as possible, but when the nose came down the nose gear did collapse. Damage was not extensive and was repaired and flew again at Luke AFB. Exact date of the mishap is unknown but believed to have happened in 1985.

Hit the ground on the Townsend Bombing Range, Georgia

Flew into the ground near Aviano AB, Italy. Pilot ejected safely. The cause was determined that both AOA heaters failed and ice formed on the probes, driving the probes full down. This caused the LE flaps to full down and the aircraft flipped on its back. The aircraft impacted a snow covered slop, inverted, and slid to a stop. The jet was enroute from Torrejon AB, Spain when the accident occured. During ejection, the pilot received a gash to his jaw from his parachute harness.

Destroyed near Luke AFB, Arizona at Gila Bend Auxillary Air Field due to an engine failure. Pilot ejected safely.

On 10 Jul 1986 the aircraft was on a gunnery training mission near the Danish island of Rømø, when there was an explosion in the engine, and smoke and flames was detected from the other planes in the formation. The pilot, Captain V.C. Boudreaux an USAF exchange pilot, was able keep the engine running long enough to get the aircraft back to Skrydstrup Air Base where he made a normal landing, and by doing so saved the aircraft. Captain Boudreaux on the 4 December 1986 received the Danish decoration 'The Medal for Superior Airmanship' by the chief of the Danish tactical air command. He was invited to the Royal palace, were the queen herself wanted to personally congratulate Captain Bourdreaux.

Collided at 14,000 feet with F-16A (78-0186, E-186) just north of Ribe during an air combat mission. The pilot, lieutenant René Sten Jensen, ejected safely.

Hit the ground near Fort Campbell, Kentucky killing Captain Ed Johnson.

Impacted the ground at Court St. Etienne, Belgium when Slt. Konrad Pauwels became disorientated and was killed

Crashed near MacDill AFB, Florida. 2nd Lieutenant Richard Satterlee was the student pilot.

Collided in mid-air with F-16A (80-3621, J-621) near Gronau, Germany. 1st Lieutenant Wiebe F.M. Schiphorst did not survive the collision and neither did the pilot of the other aircraft.

Collided in mid-air with F-16A (81-0865, J-865) near Gronau, Germany. The pilot, 1st Lieutenant Hans van Schaijk was killed in the collision.

Crashed north of Banak, Norway during a low level flight over the ocean. The crew consisted of a pilot and a higher ranking officer. The pilot experienced a sudden pitch down and due to the low level he did not have time to recover so he chose to eject. It was later speculated that the high ranking officer experienced a hart attack where he grabbed the side stick controler and pushed it forward. At the same time his body went rigid lifting up of the seat. When the pilot in charge decided to eject the seat hit the high ranking officer with extream force causing serious internal injuries which later lead to his death. The pilot in charge survived without serious injuries

Crashed into the Gulf of Mexico killing the pilot, 2nd Lieutenant J. O'brien. Probable cause is believed to be black-out

Caught fire at Hahn AFB, Germany and was destroyed on the runway. Pilot Mike Jaensch, ejected from the air plane safely.
