Experienced a fuel pump failure and ended up in the water of the Gulf of Mexico, near McDill AFB, Florida.

During a simulated emergency landing at Kleine Brogel AB, the aircraft hit the runway very hard and was severely damaged. Nevertheless the pilot managed to get the aircraft airborne again but subsequently crashed in Overpelt, Belgium. Slt. Mark Mussche from 23 Squadron ejected safely, but the aircraft was from 31 Squadron.

Ran off the runway at Leeuwarden AB. The pilot, Captain J.C. De Vries, was unfortunately killed in the incident. The accident resulted from the ingestion of a heron into the engine at high speed during a heavy weight take-off roll. Since the hook system did not function correctly, the arrestor cable was missed and the aircraft left the end of the runway with a speed of approximately 100kts. Immediately thereafter it overturned and came across a ditch which tore off the nose gear and the nose section. When the aircraft came to a halt, everything ahead of the fuel tank was gone

Collided in mid-air with Belgium F-16A (78-0156, FA-41) near Opont, Belgium. Pilot of this aircraft was Lieutenant Roger Uytterhagen

Collided in mid-air with Belgium F-16A (78-0122, FA-07) near Opont, Belgium. Piloted by Captain Michel Thibaut
10 Nov 1983 [ w/o] 82925 82- 0925 USAF 16 TFTS F-16A Block 15 K Details
Crashed on the Utah Test & Training Range killing Captain Christopher Barber. The unit was flying night mission. The pilot accidently perceived the flashing light on a train engine as the strobe of his flight lead and attempted to rejoin on the light with no radar lock.
