While taxiing at night during on a deployment to Cold Lake, Alberta the aircraft departed a taxiway and the nose gear sunk into the ground shearing it off. The aircraft is repairable.

Crashed during a training sortie 120 miles south of Goose Bay, Newfoundland. The pilot, Capt. Jac Jansen, ejected safely from the aircraft which was experiencing engine problems

Collided in mid-air with F-16A (81-0865, J-865) near Gronau, Germany. The pilot, 1st Lieutenant Hans van Schaijk was killed in the collision.

Crashed near Goose Bay, Newfoundland. The pilot, Captain Bob Verkroost, was able to eject safely. The pilot was on a low level training flight. Captain Verkroost later ejected from J-359 on September 23, 1992.

Crashed into the North Sea just off the coast off Doggersbank, Netherlands. The pilot, 1st lt. Marcel Bijmoer, ejected safely and was rescued.

Crashed near Linz, Germany in a housing area. One civilian was killed and five more injured. Unfortunately the pilot, 1st Lieutenant Peter 'Gipsy' Enzerink, didn't manage to bail out and was killed in the accident. The wingman returned home safely.

The aircraft went off the runway during taxiing at Manas AB, Kyrgyzstan. The nose gear collapsed after exiting the runway. Probable cause of the incident was that one of its brakes was broke and all the hydraulic fluid was lost. Waiting to be repaired. Supposed to fly again in August of 2005.

Crashed following an engine failure near Twenthe AB (Haaksbergen). The pilot managed to eject safely when flames were comming out of the nozzle.

When arriving at the SABCA factory in the early morning for some modifications and repainting to be done in anticipation of being transferred to Chile (it would have become airframe #745 with the FACh), the nose landing gear failed upon landing at Gosselies AP. The pilot belly landed the F-16 with him being unhurt. The airframe was briefly stored at Volkel AB for damage assessment. Although repairable it was decided not to repair the airframe due to the associated cost and simply replace the airframe for the Chilean Air Force with #J-203. Therefore this is a w/o.

Made an emergency landing at Leeuwarden AB after suffering an engine malfunction.
