Crashed after a mid-air with an F-4E Phantom II (68-0546, 222). Pilot Major Ehud Ben Amitay was killed as well as the pilot of the F-4, Dan Weiss. The navigator of the F-4, Eliezer Adar, was rescued

Lost during landing after the engine failed. Major Boaz Nehorai was killed. After the aircraft suffered an engine failure, the pilot tried to land at an abondoned airstrip in de desert. He was able to land the F-16, but couldn't stop before the end of the runway. The aircraft tumbled up-side down and cought fire. The pilot stayed with the aircraft.

Suffered a landing mishap in which the main undercarriage suffered severe damage with also the central bulkheads being damaged. It was almost declared damaged beyond repair, but since the airframe was so young, it was decided to reapir the damage, which lasted for a year.

Crashed at 13:45 hours local near the village of Bnei Haver. Dove into the ground after executing ACMI training. The pilot, Lieutenant Assaf Ramon (son of Col. Ilan Ramon), was killed in the accident. Cause of the accident was determined as G-LOC.

The aircraft crashed in the Negev desert during an instruction flight. The crash was due to a bird ingestion. Both pilots ejected with the instructor being fine but the student being badly wounded.
