Destroyed at Ørland AFB, Norway, after being 'borrowed' by a ground crew member. The aircraft was stolen by a member of the ground crew and the aircraft crashed on a farm in the neighbourhood of the village Brekstad, close to Ørland AFB. The 28 year-old military took, in absence of his colleagues and pilots, control over the aircraft, started the engine and took off, to the surprise of the Norwegian crews. Five miles from the base he crashed after being airborne for 2 minutes and 5 seconds.

Ran off the runway at Faro, Portugal after suffering a bird strike during take-off. The aircraft was on a squadron exchange with 302 sqn.

Crashed in the North Sea near the Dutch island 'Vlieland' at around 10:55am local. The pilot, Cdt. Fabrice Massaux, was killed in this accident. A four-ship formation was practising strafe runs over the Vliehors training range when one of the F-16s crashed. Circumstances are still unknown and pending investigation.

The aircraft was one of three F-16s from 10 W being flown to the Czech air base at Hradec-Kralove to take part in the Czech IAF. On arrival, in heavy rain with a lot of standing water on the runway, the leader, Lt Col R. Theys, landed uneventfully and was followed by the landing of the pair. However, as the pair touched down in the beginning of the runway, they began to aquaplane under heavy braking. With 4,000 ft of runway left the leader ordered a go-around and lit up his afterburner, throwing a wall of exhaust and water at the number two and three aircraft. The pilot, Lt Anne-Marrie Jansen (the only female Belgian AF fighter pilot), was still happy with her braking performance and decided not to go around and attempted to steer off to the left hand taxiway at the end of the runway, because there was no arrestor cable nor overrun at the runway. She was going too fast and exited the runway at high speed. This forced her to eject as the aircraft started overturning on its back, tearing off the tail fin and smashing the nose cone. The airplane never caught fire. She only suffered minor injuries.

During a functional check flight the aircraft experiences vibrations while testing the engine in region 1 (afterburner). The pilot reduced power, but the temperature continued to rise and the aircraft kept vibrating. While bringing the thrust to idle, the engine flamed out. The pilot had to restart the engine, but was unsuccesfull. He made a flame-out landing at Kleine-Brogel AB. The pilot was Cdt. Rudy 'Chouke' Schoukens.

Less than a month after delivery, the aircraft flies through some high voltage power cables in the valley of the Dordogne, Southern France. The pilot, 2nd squadron commander Cdt. Alain Letellier, succeeds in landing at Aulnat (near Clermont-Ferrand). The aircraft was heavily damaged on the belly, but was repaired

Landinggear collapse during landing at Kleine-Brogel AB. Aircraft suffered only minor damage

Collided with F-16A (80-3584, FA-93) at 11:30 hours near the towns of Somal and Failon, Belgium. Both aircraft were conducting a FAC exercise at the time of the accident. One pilot, Lieutenant Colonel Ronny Vaerten (FA-93) didn't eject and was found in the wrecked cockpit while the other pilot, Lieutenant Wouter Rombouts ejected, suffering minor injuries.

Suffered an engine flame-out. The pilot, Maj. Frank Dewinne, succeeds in an airstart and landed safely at Leeuwarden AB, Netherlands

Engine bird strike at Schleswig-Jagel AB, Germany. The aircraft was performing a fly-by for the 2004 Tigermeet. Captain Steven 'Vrieske' De Vries landed the aircraft safely.
07 Mar 1997 [ act] FA-127 89- 0005 BAF 2 W F-16A Block 15 AG OCU Details
Minor mishap involving this F-16A. Nose wheel collapsed after landing. Damage was minor and only to the nose gear door. The pilot stayed with the aircraft since it was not moving fast during the incident. The nose gear collapse took place at Florennes. The aircraft was repaired

The aircraft suffered a devastating ground firing after accidentally being fired upon from another F-16 at Florennes AB around 14:10 hours local time. A second aircraft sustained some minor damage and two men sustained some minor injuries.

Several bullets fired from another parked F-16 struck the vertical tail, fuselage and engine. The aircraft next to this one took the brunt of the destruction (FA-128) and burned up. This aircraft will be repaired.

During the Weapons Instructor Course held at Leeuwarden AB this F-16 went 'wild'. While starting up the F-16 started to roll. The pilot eventually ejected (on the ground) and the F-16 ended up in a building. It isn't clear whether this will be a write-off or not.
