Collided near Augusta, Georgia with F-15A (75-0056) of the 128th TFS 5 miles east of Wadley, Georgia. In the backseat was Bill Elliot who is a well known NASCAR driver. The missile rail on the F-16 hooked the intake on the F-15. Major Wayne F. Conroy was able to land the F-16 at Dobbins AFB, Georgia. F-16 was repaired only to be lost in a mishap on November 14, 1991. F-15 pilot, Lt. Col. Bruce W. MacLane ejected safely.

Crashed onto Utah Test Range killing both pilots, Capt. Michael Terrance Sowell and Capt. Arnold Arthur Clarke. The aircraft suffered a controlled flight into terrain (CFIT).
